Summarise shift handover notes

Turn messy end-of-shift notes into a clean handover with pending tasks and patients to watch.

Recommended: BuilderWritingDocuments

Best for

  • Ward nurses and duty doctors closing a shift
  • Wards where handovers happen on paper or in chat
  • Anyone who wants the same handover format every day

What you need

  • An AI assistant your hospital has approved
  • Your shift notes, with patient names and IDs masked
  • Your ward's handover format, if you have one

Starter prompt

Copy it, fill in the parts in square brackets, and paste it into the AI assistant your organisation allows.

You are helping a [ward name] nurse write a shift handover. Below are my rough notes from the [morning/night] shift. Patient names are replaced with bed numbers. Produce a handover with these sections: 1) Patients to watch closely and why, 2) Medications or tests due in the next shift with times, 3) Pending tasks and who owns them, 4) Anything unusual reported by family. Keep every clinical value exactly as written. Do not add diagnoses or advice. If something in my notes is unclear, list it under 'Check with me' instead of guessing. Notes: [paste notes]

How to use it

  1. 1.Mask the notes first

  2. 2.Paste and run the prompt

  3. 3.Check every clinical value

  4. 4.Save it as a template

  1. 1

    Mask the notes first

Replace names, UHIDs and phone numbers with bed numbers before pasting anything. Use only the assistant your hospital has approved for patient-related work.

  2. 2

    Paste and run the prompt

Paste the masked notes into the starter prompt and run it. Keep the four sections the same every shift so the next team knows where to look.

  3. 3

    Check every clinical value

Compare doses, times and vitals line by line with your original notes. Fix anything the assistant reworded or dropped.

  4. 4

    Save it as a template

Once the output looks right, save the prompt as your ward template. Next shift, you only paste new notes.

Watch out. Never paste identifiable patient data into an unapproved tool, and mask names and IDs every time. The handover is still your responsibility, so read it fully and verify each value before you sign off.

What you end up with

A consistent, checked handover note in the same format every shift.
